DSS now uses a more consistent assessment and allocation approach across NASCs and Enabling Good Lives sites. Where relevant to the disabled person’s needs and wellbeing, assessments can also consider family, whānau and carer needs, including caring, respite or wellbeing breaks, and support to reduce stress. MyLog helps families record the day-to-day picture before the conversation happens.
MyLog does not replace the assessment. It helps you prepare for it.
DSS’s assessment conversation leads to a My DSS Funding Plan describing what funding a person has, what it is intended for and the outcomes it is meant to support. MyLog sits on the family side of that process: recording day-to-day information, goals, patterns and relevant wellbeing information so the family has a clearer history to draw from when discussing their circumstances.
